Wafer Shipping Requirement
The following are the requirements for the packing list and pro forma invoice when making shipments to China. All items listed below must be included in the packing list and pro forma invoice.

A. Packing List Requirements

1. Company name
2. Consignee: "GEM Electronics (Shanghai) Co., Ltd."
3. Packing list number
4. Invoice number
5. Dispatch date
6. Shipping method (E.G. air freight)
7. Part number(s)
8. Description of material: "Wafer"
9. Units: "K Die" or "Die" (please state quantity of die, not wafers)

B. Pro Forma Invoice Requirements

1. Company name
2. Consignee: "GEM Electronics (Shanghai) Co., Ltd."
3. Invoice number
4. Date
5. Packing list number
6. Part number
7. Description of material: "Wafer"
8. Unit:"K Die" or "Die"
9. Quantity (please state quantity of die, not wafers)
10. Currency:US dollar
11. Unit price:"$0.20 USD/Die" (or "$200 USD/K Die" if unit is in K Die)
12. Total value per line item
13. Total shipment value
14. Terms of delivery (which party pays for freight charges)
15. Terms of payment

To expedite the receipt of shipments through China customs, please email a soft copy of the packing list and pro forma invoice to arrive in advance of shipments.

Any shipments into China containing wood must also be accompanied by a signed declaration stating that the shipment does not use unprocessed wooden packing materials.
